on resume but not on app


i was laid off xyz co. after 2 years and then 9mos. later went back to xyz co. i list on my resume the years 1998 -2002 and dont show the split in years. in those 9 mos i worked for my relative doing construction(odd jobs) before going back. after going back to xyz co i left after 6 mos to get my current job i have had for 5 years.  is that ok? 

i never thought about it until now because i got a job offer and realized i have to complete the application listing prior jobs.  how should i complete the app?

You can list the job once, but put the both date ranges:

XYZ Co: 1/98-1/00; 10/00-1/02
Your description can say that you were "recruited back to XYZ Co." - it looks good if your former employer valued your work so much that you were re-hired.

Should i just put that down on the app and not mention it then? im losing sleep over this. i am going to accept this offer but dont want to have it backfire on me down the road. logically, i dont think its a big deal. if i am asked about it i will tell the truth.  

I wouldn' t lose too much sleep over this - the position was five years ago, the company recruited you back which looks good, and you' ve held steady employment since then. I think including the two sets of dates should be fine, and you can explain what happened when asked.
