Good question. No, I never have attached it. My feeling was that some harried HR person reading over a thousand resumes will never have the time or the inclination to read it. 

Maybe if you are in an interview itself, it might prove helpful if your qualities are a fit for the position you seek. That would take some very careful thought on the part of the job candidate first. I would proceed with caution though.
