I did not work for two years due to a back injury.  What is the best way to cover this gap of time on a resume?

First of all, do not be extremely worried you've been out for 2 years, especially with your motivation. I just wanna say I've heard of even more tragical cases, when people didn't work for 13-15 years and still wanted to come back.
It is true that it is more difficult to mention this in the resume but still, just place your working periods, and, when asked, you will explain the conditions in which you were practically forced out of the ability to work for two years. Also, enhance the fact that your health is stabilized now and you are ready to work.
