What to Say on Application and inverview

I have worked for a major company for years however I am in a position where I can job post for another position.  The problem I have is that in my current position I was demoted a couple of years ago.  Now I finally have the performance number to try for another position in the company.  The problem I am now facing is how do I mention it when I apply for the the new job?  Should I mention it when I fill out my application and or my interview?  I was thinking I want to mention it before they read it in my file or hear it from my supervisor or manager (the demotion was due to performance level- not being able to process work fast enough to meet a 100-105% performance requirement).

What advise does anyone have??

In most companies before they even interview you they'll be able to see at least your most current job evaluation scores, if not the last several.  I'm betting they'll know what happened.  Be prepared to discuss it in the interview if it comes up and say something about how hard you've worked since.

I actually thinking about talking about it in my interview.  I was just wondering how in depth to mention it.  My performance this past year was good and my current supervisor and colleagues will.  I decided I don't want to name drop and blame my former supervisor in the interview since I think it will sound tacky and may come off the wrong way (even though I do think part of it was her).
