Going back to original career


Hi there - not sure if this is where I should post my question, but here goes!

I successfully changed careers 5 years ago from the healthcare field into higher education. It was a perfect fit and I enjoyed the work I did immensely. However, I have chosen now to stay home with my children and would like to find part time work in my original career (occupational therapist). I have a 5 year gap of clinical experience now on my resume, but have had 5+ years of clinical experience prior to the career change. My question is how to address this on my cover letter (if I even need to?) and how to highlight my clinical skills on my resume (since the work history as an occupational therapist is on the bottom chronologically!). I have had success with a functional resume, but am not sure that this is the right situation to use it again...
